SPECIAL MEETING NOTICE
As a result of the arbitration ruling, a meeting will be held at 1:00 p.m. on February 11, 2012 at the Union Hall, 1414 N. Memorial, Tulsa.
You will need to bring the following documents to this meeting:
A. Requested benefit commencement date.
B. Marital status at benefit commencement date. If married:
a) Name of spouse
b) Social Security Number of spouse
c) Spouse's birth date
d) Copy of marriage certificate
e) Copy of QUADRO (Qualified Domestic Relations Order)
C. Current contact telephone number and address
D. Copy of birth certificate
E. Copy of spouse's birth certificate
Please pass this information to all Bridge people, so we can make sure all are in attendance at this meeting.
